Civil Rights Compliance Checklist

This form is completed annually by all contracted service providers and
submitted to the contract manager who then submits the form to their EEO
Coordinator by July 15 of each year. This form is also completed by each county
health department, hospital, local program and program office and submitted to
their EEO Coordinator by July 15 of each year. This form may be saved as a word document and then mailed or emailed to the appropriate individual.
Discrimination Complaint Form

This form may be completed, signed and submitted by employees and applicants for
employment to file a Civil Rights Complaint with the Department of Health, Office
of the Inspector General. Employees and applicants for employment must file within 365 days of the alleged discriminatory action. Clients and applicants for services
may file a Civil Rights Complaint with the Department of Health, Office of Equal
Opportunity and must file within 180 days of the alleged discriminatory action.
