Care CoordinationChildren who have special health care needs often require more health care services than typically healthy children. Getting the necessary services to maintain your child's health can involve dealing with many different health care providers, state agencies and local community resources. Managing the health care system can sometimes be confusing and overwhelming for parents who are trying to cope with their child's complex medical issues. That is why Children's Medical Services offers care coordination services to help guide families through the health care system. CMS care coordinators are nurses and social workers who work with the child's family and doctors to identify needs and the resources available to meet these health care needs. Every child enrolled in CMS is assigned a care coordinator to make sure that your child gets the services needed. Care coordinators help your family make and follow-up on doctor visits, medical tests, or procedures and may also assist in getting behavioral, developmental, and social services.
