| Position Description:
Targeted Case Management position requiring home and field visits
Coordinating services for children birth to 36 months and their families
Providing support and appropriate referrals to families identified on case load
Participate in team meetings and monitor delivery of services
Ability to work independently and as part of a team
Good organization and documentation skills
Ability to speak and write in Spanish is a plus but not necessary |
| Requirements:
Minimum one year experience working with children birth to 5 years
Minimum of a BA or BS in related field
Basic computer skills- we utilize an electronic data system and electronic case notes (Word and Excel)
Reliable transportation
Ability to work Monday-Friday from 8am-5pm
