Indicator DescriptionsIndicator 1: Compliance with this indicator is measured by reviewing child records to determine the number of days from the date that the family consented to the service or the start date of the authorization period listed on the Individualized Family Support Plan (IFSP) to the date that the service was initiated. Timely service delivery is defined by Early Steps policy to be as soon as possible, but within 30 calendar days, unless there is documentation of a child or family related issue or natural disaster which caused the Indicator 2: Performance with this indicator is derived from the services identified on the IFSP and entered into the Early Steps Data system for infants and toddlers who were enrolled on October 14, 2010. Indicator 3: Infants and toddlers demonstrate improved outcomes
Indicator 4: Family responses to a Family Participation Survey are the basis for measurement of performance on this indicator. Indicator 5: Performance with this indicator is measured by comparing the number of children enrolled with an IFSP on October 15, 2010, to the estimated number of children aged birth to one in the state or service area of the local Early Steps. Indicator 6: Performance with this indicator is measured by comparing the number of children enrolled with an IFSP on October 15, 2010, to the estimated number of children aged birth to three in the state or service area of the local Early Steps. Indicator 7: Compliance with this indicator is measured through review of child records to determine if the evaluation and assessment and initial IFSP meeting were held no more than 45 days from the date of the child's referral to the local Early Steps. Indicator 8A: Compliance with this component of the transition indicator is measured through review of child records to determine if the Individualized Family Support Plan (IFSP) addresses steps and services to help the child and family transition to the Local Education Agency or other service provider at age 3. Indicator 8B: Compliance with this component of the transition indicator is measured through review of child records to determine if the Local Education Agency was notified of potentially eligible children. Indicator 8C: Compliance with this component of the transition indicator is measured through review of child records to determine if the transition conference was held at least 90 days prior to the child's third birthday. |
