Requirements for Death Certifications:
Holmes County 2005 to present

• Fee for Birth Certifications: $10.00
• Issued only to the person named on the record when 18 years of age or older, or to the registrant's parent, legal guardian
• Legal guardian must show guardianship papers
• Must have a valid picture ID
• Payment in cash, check Visa, Mastercard or Debit.
• Fill out Application for Birth Certificate

• Fee for Death Certifications: $10.00
• Must be a family member of legal age or an attorney (if cause of death requested)
• Must have a valid picture ID if cause of death is requested. 
• Payment in cash, check Visa, Mastercard or Debit.
• Fill out Application for Death Certificate
