Immunization Requirements for School Entry - 2012-2013
The Martin County Health Department is providing immunizations on a limited basis. The Health Department will provide a seasonal “Back to School” and “7th Grade/Tdap” immunization clinic as well as community response to communicable disease outbreaks.

Please contact your private health provider to schedule an immunization appointment and obtain a #680 form, if needed.
Children who do not have a primary health care provider, may contact Florida Community Health Centers (FCHC) at 772-597-3596.
