Immunizations

Immunizations for School Age Children:
 
Proof of vaccinations, with form #680 is required for entry into school. 
 
Immunization Requirements for School Entry - 2012-2013
 
The Martin County Health Department is providing immunizations on a limited basis. The Health Department will provide a seasonal “Back to School” and “7th Grade/Tdap” immunization clinic as well as community response to communicable disease outbreaks.


680 Forms

 

Please contact your private health provider to schedule an immunization appointment and obtain a #680Picture of family form, if needed.

 

Children who do not have a primary health care provider, may contact Florida Community Health Centers (FCHC) at 772-597-3596.

 

The Health Department can provide #680 forms for a $10 fee. Call (772) 221-4000 for more information.


Adult Immunizations


Adult immunizations including flu shots and international travel immunizations are available through local community providers.
