Birth/Death Certificates

Birth Certificates:

 

We offer birth records from 1930 to present. Records on birth events that occurred prior to 1930 can be obtained from the Bureau of Vital Statistics.

 

Birth records under seal by reason of adoption, paternity determination or court order must be obtained from the Florida Bureau of Vital Statistics. For a record under seal write to:

Death Certificates:

 

Death certificates for deaths occurring in Martin County are available only at the Stuart office from 2010 to present. For death certificates prior to 2010, contact the Bureau of Vital Statistics at the above address, or VitalChek at their website: www.vitalcheck.com or by calling 1-877-550-7330.
 
You must be a family member or legal representative when requesting cause of death.

 

**A legible copy of a valid driver's license, passport, state or military identification card must accompany all requests.**

A completed application should be submitted along with the appropriate fee. Make check or money order payable to the Martin County Health Department. Please do not send cash.

The Florida Department of Health has partnered with the VitalChek network to process online, credit card orders.
