bullet How do I get my immunization records?
If you received your immunizations in Florida, you can visit any of our immunization clinics during their clinic hours and request your records. You don't need an appointment.  There is no charge for this service.  Please consult our immunization clinic web page for clinic locations, hours of operation and phone numbers.
 
bullet How do I report a problem with food, or unsanitary conditions where food is served or sold? Who do I call if I got sick at a restaurant?

bullet Does the Volusia County Health Department inspect restaurants or hotels / motels?  Where do I get information on restaurant and lodging inspections? What do I do about reporting bedbugs at a local hotel/motel?
The Volusia County Health Department does not do restaurant nor lodging inspections. They are done by the Florida Department of  Business & Professional Regulation:

bullet Does the Volusia County Health Department provide the vaccine for shingles?
Yes, the Volusia County Health Department does provide the vaccine. The cost is approximately $170. Please call ahead to make sure that the vaccine will be available when you visit.  Please consult our immunization clinic web page for clinic locations, hours of operation and phone numbers.
 
bullet Does the Volusia County Health Department give out free condoms?
Yes, we do give out free condoms at each clinic location in DeLand, Deltona, New Smyrna Beach and Daytona Beach. For any questions about condom pick up please call 386-274-0651.

 
bullet Where do I get a copy of my marriage or divorce certificate?
The Volusia County Health Department only issues Birth and Death Certificates.  For marriage or divorce certificates please see the Florida Department of Health website:

bullet Does the Volusia County Health Department accept credit cards?
Yes we do, both credit and debit. Credit cards accepted are Visa, Mastercard, American Express and Discover.  We also accept debit  cards but with the Visa or Mastercard logos only.
