Health Fair/Community Event Requests

The Volusia County Health Department participates in a variety of health fairs and community events throughout the year. To request the Volusia County Health Department to participate in your community event, complete the request form.

Please note the following:

• Participation in community outreach events is based on staff availability and focus areas of our programs.
• Once you have completed and submitted the request form, a staff member will contact you.
• If you have additional event documentation to provide (ie. flyers, registration forms) please email them to us using the link below:
  
  Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to this entity. Instead, contact this office by phone or in writing
