Childhood Shots / Immunizations (Ages 0 - 18)  - Locations, Hours, What to Bring

(Immunizations for Ages 0-18 years)

What you need to bring:

• A copy of the child’s immunization record
• Parent must accompany the child.
• If parent or guardian is unable to accompany the child another adult may bring the child.  The adult must present a written consent from the child's parent or guardian. The written consent must contain:
  • current date
  • child's name
  • child's date of birth
  • parent's signature
  • parent's contact phone number
  • Form must be notarized
     

Please note we are closed for the following holidays - New Year's Day, Martin Luther King Day, Memorial Day, Independence Day, Labor Day, Veteran's Day, Thanksgiving Day, Friday after Thanksgiving and Christmas Day.  If the holiday falls on Saturday, we are closed the Friday prior to the holiday.  If the holiday falls on Sunday, we are closed the Monday following the holiday.
