Please fill out the below online pre-registration form and the Anti-Tobacco Education Classroom Rules (.pdf - 115kb). Both a parent/guardian and student signature acknowledging the rules is required for the student to participate in the class. The Anti-Tobacco Education Classroom Rules document must accompany the student on the day of the Anti-Tobacco Education class. By filling out and submitting this form, your child will be enrolled into the next Anti-Tobacco Education Class. The Anti-Tobacco Education Class is customarily** held on the first Tuesday of each month. An e-mail containing the Tobacco Prevention Program information will be sent out to the e-mail address provided that will provide all necessary information. If an e-mail address is not provided an information packet will be sent out to the address provided. ** Class dates are subject to change
