For security reasons, when a user attempts to login to
the system incorrectly 3 consecutive times, the user’s account
gets locked automatically. The user must contact the key
user of his/her agency and request the account to be unlocked.

In order to change your password, you would first have
to pass the provider login screen. On successful authentication,
the user login screen will be displayed with the ‘Select
to change password after login’ checkbox. You will have
to enter your correct username and password and select the
checkbox to change the password. On selecting the ‘Login’
button, you will be taken to the ‘Change Password’ screen
where you can change your password.

You have to contact the key user of your agency if there
are any changes to your profile information like email,
phone number etc. You can view the key user directory and
search by agency name to view all the key users of your
agency and their contact information.

You can view a complete listing of all the key users
of your agency in the Key User directory. In order to the
get to the key User directory, you will first have to logon
to the EMSTARS application. Once you logon, select the ‘Agency’
tab and choose the ‘Key User Directory’ option in the left
pane. You can then search by agency name to view all key
users and of a particular agency and their contact information.

Yes, where possible. We recognize that several circumstances
may inhibit collection of SSN in the field, such as a foreign
visitor or illegal alien, infants, John Doe's, or the SSN
may just not be readily available. There are field values
available to accommodate each situation (no blanks will
be accepted). However, every attempt should be made to collect
and record the patient's SSN, either at the time of service,
or updated after the fact. This is an important factor for
billing; but it is also the only method available to link
patient outcome data from hospitals and rehab. The quality
and quantity of an agency's outcome data will be directly
related to their ability to collect and report patient SSN.
