County Grant Information

Note: please scroll down to see matching grant information.

We are pleased to announce that you may now apply for your county's annual grant from the state for the improvement and expansion of your county's prehospital emergency medical services (EMS). You can see the amount for your county this year by using the following link.
FundsForCounties

As required by Chapter 401.113 (1), Florida Statutes, these funds are to be used solely to improve and expand prehospital EMS, which does not allow costs for replacement or ongoing expenses. No recurring dues or payments of any kind are allowed without written consent from the state EMS Grants program.

We are again using the 2008 edition grant booklet and forms. If you need a copy, you may access the PDF form online at http://www.fl-ems.com/Forms/Forms.html or request forms in Microsoft Word from the state EMS office.

To obtain its funds, the county must complete and send to the state the signed original and one copy of the (1) two-page application form DH Form 1684, (2) Request for Grant Fund Distribution page DH Form 1767P, and (3) resolution described in Item #4 of the application form. Your budget must total the amount referenced for your county.

Send to: Attn. Alan Van Lewen, DOH EMS Program County Grants, 4052 Bald Cypress Way, Mail Bin A22, Tallahassee, FL 32399-1722.

The deadline for the state EMS office to receive completed applications is November 15, 2013.

If you have any questions, please contact Alan Van Lewen at (850) 245-4440, extension 2734.
