Civil Rights Compliance Checklist (doc - 98.0kb)
This form is completed annually by all contracted service providers and submitted to the contract manager who then submits the form to their EEO Coordinator by July 15 of each year. This form is also completed by each county health department, hospital, local program and program office and submitted to their EEO Coordinator by July 15 of each year. This form may be saved as a word document and then mailed or emailed to the appropriate individual.
