Update Your Practitioner Profile -
Florida licensed practitioners who used CoreSTAT to update their
practitioner profile, can now update their profile information
online by visiting FLHealthSource.com. Access to the profile update system
will require the practitioner enter a user identification and
password issued by the Department with their renewal application.
Practitioner's who are not currently in a renewal cycle can obtain
their user identification and password with one of the following
options:
- If your email address is current with the Department, follow
these easy steps:
1. Go to FLHealthSource.com.
2. Click on "Update My Profile" listed under Licensee Services.
3. Select "forgot/user id and password"
4. Select your profession and input your license number.
5. Click "Process"
6. Select your current email address
7. Click "Process" and your user identification and password will
be emailed to the selected email address.
(Note: Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to this entity. Instead, contact this office by phone or in writing.)
- If we do not have your current email address, please e-mail
the Department at Licensure_Services@doh.state.fl.us or call (850) 488-0595.
Please provide your first and last name, the last four digits of
your social security number, profession and license number for
verification purposes.

p.m., Monday through Friday, Eastern Time, with the exception of
