***Please note the application is for licensure. For the issuance of a temporary permit, a request for a temporary permit and the scheduled date of examination must be entered on the application OR followed up by a written statement (email or fax), and the licensure file complete before a temporary permit may be issued. The initial review of the application will be completed within 30 days of receipt of an application in the Department, by application processors. Application status' and receipt of supporting documents are NOT available until the initial review has been completed. You will be notified immediately upon the initial review of your application status. If your file is complete at the time of the initial review the Temporary Permit will be issued at that time.

The $180 fee must accompany the application or the application will not be processed.

The licensure fee ($75) may be refunded to you if you are denied licensure or if you decide to withdraw your application.
