Provisional Speech Pathologist or Audiologist Licensure Fees

Application Fee	$75.00 (non-refundable)
License Fee (issued for 18 months)	$100.00
Unlicensed Activity Fee	$5.00

Application, License and Unlicensed Fees are due at the time of application.

Initial Licensure Fee is refundable if applicant is found ineligible for licensure. Applicant must request refund in writing.

Applicants and licensees are responsible for the fees outlined in applicable board or department rule at the time an application is received.
