Developing a Community
Health Improvement Plan
Directions
Now that you have identified the strategic
issues you will be working on for the next few years in your community, it
is time to document these issues, goals and activities in a Community
Health Improvement Plan document. A community health improvement plan is
a compilation of individual action plans.
For this exercise, choose one of the three strategic
issues identified by your group. Using the information you recorded on the
Strategy Development Matrix
Worksheet
in the previous section, you will now record the information on the
Template
for the (Your County Name Here)County Health
Improvement Plan  .
The next step is to brainstorm with your partners to develop
objectives for the identified strategic issue. You will be developing no
more than three outcome (long term) objectives for each strategic issue.
You will then go on to develop impact and process(short
term) objectives.
For those counties/communities using the
Sterling Model, a line is includedon the action plan
template to document which Sterling Criteria the strategic issue
addresses.
The example

serve as a reference during this exercise.
Responsible or lead persons or agencies,
target dates as well as any comments should also be recorded on the
community health improvement plan template.
You may refer to the
