- Apply tools and techniques to collaborations towards alignment and achievement of shared health outcome goals
Mobilizing community partners to identify and solve health problems is among the Ten Essential Public Health Services. Building partnerships is a foundational step in community health assessment and health improvement planning as well as for other community-based public health interventions. The National Association for County and City Health Officials’ (NACCHO’s) Mobilizing for Action through Planning and Partnerships (MAPP) strategic community health planning model includes partnership building among its key elements in organizing for successful community-driven health improvement efforts.
The modules below are based on materials from the Partnerships for Improving Community Health regional meetings held around the state in 2009 and current literature (see bibliography). In addition to the presentations, tools and links to other resources are included for each module.
