The Community Health Status Assessment (CHSA) answers the question, "How healthy are our residents?" and "What does the health status of our community look like?" The results of the CHSA provide an understanding of the community's health status and ensure that the community's priorities include specific health status issues.

Directions

  1. Using a county health status profile report from Florida CHARTS  www.Floridacharts.com write a brief description of each of the categories of health issues listed in the tables on the "County Health Profile Domain Worksheet". 
  2. Prepare a brief summary of the health status of the county you selected describing the current health status and possible contributing factors.
