Obtaining a User Account for EDRS

Who can get an authorized user account for EDRS? 

Any funeral director licensed to operate in the state of Florida and their currently employed staff members OR any private physician licensed to practice in the state of Florida and their currently employed staff members.  All requests for user access to EDRS is verified against databases maintained by the state of Florida.  Anyone attempting to gain unauthorized user access may face severe penalties or consequences.

  • Citrix Access Form:

    • Used to gather information pertinent to creating your user ID.  This form is used for both Electronic Birth Registration and Electronic Death Registration access.  Please pay close attention to the form when completing it and before submitting it to the state office.

  • Computer Use and Confidentiality Form: 

    • To acknowledge your complete understanding in regards to confidential information and the criminality associated with any misuse or improper release of that information.

  • Once you have submitted the access forms, you are required to complete the online tutorials before you will be granted full access.  Those tutorials can be found at the link below.
