COMMUNITY REINTEGRATION AND LONG-TERM
COMMUNITY-BASED SUPPORTS COMMITTEE
Goal:
To identify and
develop community resources and partnerships to ensure individuals
with brain and spinal cord injuries are able to live independently
in a safe and healthy environment in the community.
Committee functions shall include
but not be limited to the following:

Monitor and evaluate the performance and activities of the following
programs and initiatives:
o
Medicaid Home and Community Based Waiver Program
o
Consumer Directed Care Program
o
Spinal Cord Injury Peer Mentoring Program
o
Family Community Support Program
o
Institution Transition Initiative
·
Develop collaboration and integration of efforts with community
partners and agencies to provide more cost effective and quality
long-term community-based services and supports.
·
Identify housing and transportation issues and needs and collaborate
with FAAST, the Public Housing Authorities and Transportation
Disadvantaged to make recommendations to resolve the issues and
needs identified.
·
Identify recreational and wellness issues and needs and make
recommendations for resolution.
·
Identify employment issues and needs and collaborate with Vocational
Rehabilitation of the Department of Education to make
recommendations to resolve the issues and needs identified.
·
Evaluate the impact of aging on client needs and program resources
and advise the Council and Bureau on revisions to policies,
procedures, or statutes to meet the needs and obtain program and
community resources.
·
Assist the bureau with
the recruitment and retention of Medicaid Waiver Providers and
mentors.
·
Monitor and evaluate
Consumer and Stakeholder Satisfaction and make recommendations to
the Council and Bureau to resolve issues identified.
