State Primary Care OfficeWaiver Programs for Foreign Medical Graduates J-1 Visa Waiver State Conrad 30 ProgramThis program allows physicians to obtain a waiver of the J-1 visa two-year home residence requirement. Physicians must provide clinical care full time in a federally designated shortage area (HPSA) for a three year period. Thirty (30) waivers will be recommended from October 1-September 30 of each year and the deadline for submitting applications is the first Monday in November. The state's current priority is primary care physicians in the highest need areas.

National Interest Waivers (NIW) National interest waivers are available for physicians of exceptional ability that show it is of national interest he or she remain in the United States. Physicians must provide clinical care full time in a federally designated shortage area (HPSA) for a five year period.
HHS Exchange Visitor Program This program allows physicians to obtain a waiver of the J-1 Visa two-year home residence requirement. Physicians must provide primary clinical care full time in a federally designated shortage area ( HPSA score of 11 or higher ) for a three year period. Additional requirements are outlined in the guidelines below.
Transfer Request All physicians ( J-1, H-1B, NIW) wishing to transfer to a new employer or practice facility must receive written approval from the department prior to the transfer.
