Immunization Record
Requirements for Schools, Child Care Facilities,
and Preschools
All children should begin immunizations at birth or age two
months, and follow an immunization schedule as determined by your health care
provider. If your child is behind on immunizations, call your doctor
or clinic today to get them up-to-date. Children entering kindergarten,
preschool, seventh grade, or a child care facility must have documentation of
certain immunizations (as shown below).

- Hepatitis B
- Varicella (effective 2001-02 school year)
