The MomCare Program

MomCare is a Medicaid-funded program authorized by a special waiver from the federal government called the Sixth Omnibus Budget Reconciliation Act (SOBRA). MomCare was developed as a partnership between the Florida Association of Healthy MomCareStart Coalitions, Florida Department of Health, Florida Agency for Healthcare Administration and Florida Department of Children and Families.

Once accepted into the program, clients will receive guidance selecting a prenatal care provider, assistance scheduling initial prenatal visits, and information about state programs for which they may be eligible. The goal of Florida's MomCare Program is to improve birth outcomes and reduce infant mortality rates through simplified application, guidance, education and care coordination services.

How Do I Apply?

MomCare is available to women who meet the following requirements:

  • Have proof of a positive pregnancy test on a physicians or pregnancy centers letterhead that states expected delivery date.
  • Have a household income between 150% and 185% of the poverty level. See Income Chart.
  • Be a United States citizen or a legal alien.
  • Be a Florida resident.

Filling out the Application:
  • Fill out applicant information in full. Answer questions 1-10 completely. Do not omit ANY information or skip ANY questions or approval of the application will be delayed or denied.
  • Don’t forget to sign the form!
  • Attach your positive pregnancy test results received from a health professional's office to the application. NOTE: Results must be on official letterhead.
  • Affix postage and mail or fax the application to the Department of Children and Families (DCF) address for the home zip code of the applicant.  See the list of DCF Economic Self Sufficiency Services Centers for addresses and fax numbers.
Additional Considerations:
  • Telephone: Please list a phone number and any alternate phone numbers where you can be reached.
  • Other Income to Report: Social Security Numbers of the applicant, and the husband or father of the baby—if he lives in the home with the applicant. PLUS, any Social Security or Child Support Income received for the applicant’s other children in the home, if applicable.
  • If the applicant is under the age of 21 and resides with her parents, be sure to list the names of the parents on the application.
MomCare’s Application Process:

Medicaid eligibility is determined at DCF:

  1. Applicant completes simplified form declaring income and proof of pregnancy
  2. Local DCF office determines eligibility
  3. DCF notifies Consultec (fiscal agent)
  4. Consultec notifies the Healthy Start Coalition
  5. Consultec sends printed information about MomCare to the new client
Initial contact with the new client:
  1. Maternity Care Advisor (MCA) explains the program
  2. Client gives consent and is given notice of privacy practices
  3. A list of provider choices is given to client
  4. MCA assists the client with selecting a healthcare provider
  5. Assistance is offered in scheduling first or next appointment.
  6. Client is encouraged to complete Healthy Start screen
  7. MCA assists with WIC process
  8. Client advised of right to change provider
  9. Client advised of grievance procedures
