Prescription Assistance Program

The DeSoto County Health Department is now offering free or reduced cost medication. This service seeks to help the deprived of any age or race, those with no insurance or inadequate insurance, and those who do not qualify for other programs. Our Staff will assist you in working with your Doctor and the drug companies to help you qualify for long-term prescription assistance directly from the manufacturer of your medicine.

Qualifying

First, we must determine if you are eligible for prescription assistance. Our staff will perform a preliminary screening to ensure that you have all the necessary paperwork.

This is what you will need to qualify:
  • Photo ID and proof you are a resident of DeSoto County (if not listed on your ID or Driver’s License)
  • Written prescription(s) from your doctor or medication bottle(s)
  • Proof of Insurance coverage (if existing), Medicaid denial letter
  • Proof of Income (for all in the household), 4 Consecutive pay stubs, Bank Statement, W2 tax Form, Workman’s Compensation Statement, Food Stamps Statement
    Income Tax Statement, if you possess one

If you do not bring these items, we will have to reschedule your appointment. Some of our procedures are dictated by legal requirements, so we cannot make exceptions.

If we find that you are not eligible for this service, we will try to refer you to one of our community partners that may be able to assist you.

If your situation changes, you are welcome to return to us for a re-evaluation.

Refill Policy

It is the policy of the Prescription Assistance Program to require you to call our service or leave a message when you receive your medication. We need to hear from you in order to refill you medication. Please call for a refill when you have at least a 30 days supply left. It is your responsibility to inform us if you do not hear from your doctor’s office or receive your medication. If you do not pick up your medication and contact us, we will no longer be able to provide assistance.
