Immunizations

Immunizations - Image of a syringeThe DeSoto County Health Department offers various immunizations (vaccines) at our primary care facility located at 519 West Hickory Street.

Child must be brought in by custodial parent or have a Consent for Services Provided to Minors Form (English  -  Spanish) from the parent with a phone number to contact parent on that day, along with the child’s complete immunization shot history.  This form must be notarized. Also, if this is your first visit to DCHD for immunizations, please bring child's social security card and past shot records.

The immunizations offered at DCHD include:
  • Influenza (Flu)
  • VZV (Chickenpox)
  • Pneumonia
  • Tetanus
  • Hepatitis A & B series
  • Measles, Mumps and Rubella (MMR)
  • Human Papillomavirus (HPV)
  • Meningitis
  • Polio (Children Only)
  • Shingles & HIB

All CDC recommended childhood vaccines are offered without charge to any school-aged child (through the age of 18)
