Infant Screen

The Healthy Start Infant Risk Screen is a short questionnaire that identifies risk factors that could affect you or your baby’s health and development. It is completed by the hospital or birthing center where you delivered you baby. Midwives may also complete the Infant Risk screen. If one was not offered to you, ask for it.
