HEALTH DEPARTMENT SERVICES
Appointments for all services can be made at the appointment desk by calling (352) 540-6800. Currently, mental health services are available at the Nature Coast Community Health Center (NCCHC)/Health Department for Primary Care clients referred by our providers. Communicable Disease Treatment, Education and Prevention Dental Our School-Based Sealant Program provides dental sealant services free of charge to eligible 2nd and 7th grade school children at their schools, during school hours. Dental sealants protect permanent molars from cavities. Dental services are available by appointment only at both locations. Diabetes Self-Management Education The Hernando County Health Department/Nature Coast Community Health Center has partnered with Brookville Regional Hospital to provide a community diabetes education program. Classes are held over a five week period covering topics to help the person with diabetes understand their disease and learn how to control their blood sugar. The classes are instructed by staff from both facilities and offered as part of the Diabetes Rehabilitation Program at Brooksville Regional Hospital. The program is recognized as meeting the National Standards for Diabetes Education by the American Diabetes Association. For more information on this program please contact Dee West @ 352-797-4616. Family Planning Healthy Start Immunizations Immunizations are available at our clinics from 8:00am - 4:00pm on Mondays, Tuesdays, Wednesdays and Fridays. Both offices also offer immunizations on Thursdays from 8:00am - 11:30am. Although walk-ins are welcome, appointments are encouraged to better serve the community. Brooksville clinic, 300 S. Main Street Prenatal Service Provides routine medical care to non-high risk pregnant women between the ages of 15 and 35. Services include pre- and post-partum visits and lab work. Primary Care The following Application Packets contain all the forms needed to complete and return in order to have your Primary Care eligibility processed. These packets aslo include documents that must be completed and returned at the same time. If they are not completed in their entirety, your application for Primary Care will not be processed.
