ADULT HEALTH
Primary Care
This program ensures basic health services for adults who are uninsured, or under-insured and do not qualify for federal assistance programs. Primary health care is provided for conditions within the scope of assistance that the Health Department / Health Center is qualified to provide. A team of providers and case managers helps ensure access to preventive health services, which reduce costly misuse of emergency departments.

Chronic Disease Programs
Medications for diabetes, epilepsy and rheumatic fever are available. The Health Department / Health Center also screens for diabetes, cholesterol, cancer and cardiovascular risk factors. Chronic disease screenings are provided at both our clinics and at community centers, churches and other easily accessible locations.
Medical Nutrition Therapy
This program helps individuals better manage a diagnosed medical condition such as diabetes or hypertension through individual and group nutrition counseling. A Licensed, Registered Dietician provides diet assessment, referrals, information and follow-up care. The program is open to all persons. Referral from the primary health care provider is required.
Dental
The Dental Program provides preventive, restorative, extraction, and root canal services to people ages 3 and older, who meet the economic eligibility guidelines, or who are eligible for Medicaid. Oral health presentations are available for Headstart, grade schools, and other local groups by request.
Dental services are available by appointment only.
