Vital Records, The Office of Vital Records helps you obtain certifications of birth or death, and allows you to request a birth or death certificate.

You may apply for certifications of birth and death in person. If you choose to mail in your application, please address it to the Hernando County Health Department, 300 South Main Street, Brooksville, FL 34601.

Birth Certificates:
The Health Department / Health Center can issue birth certificates for the years 1930 to present, within the State of Florida.

Birth Certificates are needed for:

  • Admission to public school
  • To obtain a driver’s license
  • A work permit
  • The right to vote
  • Eligibility for retirement
  • Social Security benefits
  • To prove citizenship
  • To obtain public assistance for dependent children

Birth Certificates can be issued to:

  • Parents
  • Individual 18 years of age or older, with proper identification
  • Legal guardians (with proper documentation, i.e., driver's license, photo ID, guardian paperwork, etc.)

Fees:

  • $12.00 for the first copy
  • $10.00 for additional copies for the same person purchased at the same time (Effective November 1, 2008)

To request a birth record from the State Office of Vital Statistics in Jacksonville, phone (904) 359-6900 or send a written request to:

Include the following information:

  • Name of Child
  • Date of birth and county of birth
  • Mother’s maiden name and father’s name
  • Relationship to the person on the birth certificate
  • A photocopy of driver’s license for identification should be included

Death Certificates:
The Health Department / Health Center issues death certificates for the years 1994 to the present for those individuals who have passed away in Hernando County. Death Certificates for other years or counties must be requested from County of death or Jacksonville Office of Vital Statistics. Death certificates with cause of death can be issued to the family of the deceased or to a licensed funeral director.

Fees:

  • $10.00 per copy (Effective November 1, 2008)
