 Diabetes Self-Management Education Program

Diabetes self-management education (DSME) is the process of
teaching people to manage their diabetes.

Our team will work with you to make your diabetes care plan:

  • A meal plan
     
  • A physical activity plan
     
  • A plan for how and when to check your blood sugar
     
  • Your personal blood sugar goals
     
  • Other health goals (such as managing your weight and blood pressure)
     
  • A schedule for regular health check ups
