  2. Have you had a hysterectomy or a tubal ligation?

If you answered no to the questions above and wish to receive family planning services to delay pregnancy,

√  You may be eligible for extended Family Planning Benefits through the Family Planning Medicaid
     program.  This program will provide you with special Medicaid benefits.  Under this program,
     Medicaid will pay for:

  • Physical exams including a pap smear
  • Birth control supplies such as pills, depo, condoms, and diaphragms
  • Pregnancy tests and limited treatment for sexually transmitted diseases

√  If determined eligible, you can receive these family planning services from any Medicaid provider,
    such as a birthing center, certified nurse midwife, county health department, doctor, family planning
    center, nurse practitioner, physician assistant, or rural or federal health clinic.
