MEDICAID FOR PREGNANT WOMEN

 

Jackson County Health Department (JCHD) is able to assist you with obtaining immediate Medicaid IF you are qualified.  This applies only to pregnant women.  To determine if you are eligible, you must come to JCHD and submit a proof of pregnancy statement that has your expected due date (EDD) which has been determined by a medical professional and photo identification.

Your application will be processed and you will obtain immediate notification of program acceptance or denial.  If you have already placed an application on line, we will not be able to process your application.  To support your application being processed quickly, you may also bring proof of the last four weeks of pay for all individuals in the household and a copy of your social security card.

You may print the form from the link below or we will provide you with one upon coming to the Health Department.  Appointments are preferred but not required.

Simplified Eligibility for Pregnant Women (SEPW): A simplified "full coverage" for pregnant women only.  Please complete the one page application linked below.  Print the application and bring it with you to the Health Department.
