Clinic Services
We offer a variety of healthcare services to meet the needs of the people in the community.
Child and Adult Health:
- Services include school entry, sports and employment physicals, acute and chronic disease management, smoking cessation and insulin programs.
- We strive to provide quality healthcare to the members of our community.

Prenatal Care:
- Services include pregnancy testing, medical exams, prenatal care, parenting and nutrition classes. We also assist pregnant women apply for Medicaid.

Family Planning
- Our family planning clinic offers a wide variety of healthcare services for women, including yearly exams, birth control, counseling and supplies. These services are based on the sliding fee scale.

Immunizations
- Immunizations are free to children birth through 18 years of age (up until their 19th birthday) from the Vaccines for Children Program. Immunizations should be started at 2 months of age. If your child has had immunizations from another provider, a copy of his/her shot history is required. Immunizations are required upon school entry.
- No appointment is needed.
- Clinic hours are Monday-Friday 8:00am to 11:00am and 1:00pm to 4:00pm.

Communicable Disease
- Services include testing, diagnosing, counseling and treatment for HIV/AIDS, Tuberculosis, and Sexually Transmitted Diseases.
- The Health Department is also responsible for the surveillance and investigation of reportable diseases or outbreaks in the community.

Payment
Personal health services are provided on a sliding fee scale after required documentation is provided. Fees vary according to the service provided. No one is denied medical services due to an inability to pay.
Acceptable proof of income can be copies of any of the following:
- The most recent month’s pay stub for all working members in the home.
- Most recent years tax return
- A notarized letter from your employer stating hours worked and wages paid before taxes.
- A tax form stating that no taxes were paid (form T4506).
- Social Security, Pension or Railroad retirement statements.
- Any other proof of income
Medicaid and Medicare are accepted, however, your proof of income must accompany your Medicaid card.
