Lee County Health Department Insulin Program

Information for people with Insulin Dependent Diabetes

 

If you do not have insurance, Medicaid or Medicare and would like to apply for the State of Florida Free Insulin Program, please follow this guideline:

 

  • Obtain the original prescription from your medical provider
  • Take the prescription with you to Lee County Health Department for the Eligibility Determination process.

 

You will need to bring with you:

 

§         Picture Identification

§         Proof of income, if possible (4 pay stubs or a statement from employer)

§         Proof of residency (a received mail item addressed to you)

 

Once your eligibility is determined and you qualify for the program, you will be seen by a nurse to:

 

§         Complete the Program Application

§         Complete the Medical Profile (please bring a list of any medications that you are taking)

§         Receive health counseling to support you in achieving the best health outcome.

Monday through Thursday: 9-11 am or 2-4 pm and Friday:  2-4 pm
