Birth/Death Certificates


The Levy County Health Department serves as the county's vital records office for birth and death certificates. For certificates from other states visit the State Office of Vital Statistics.


The Health Department is required by state law to restrict access to birth certificates and death certificates (with cause of death), to specific individuals. This law protects the privacy of sensitive information and prevents identity theft. Death Certificates without the cause of death are public records. See the information below for required documentation to obtain birth and death certificates.


Birth Certificates

The State Office of Vital Statistics maintains the original copies of the birth records from all counties within the State of Florida. The Levy County Vital Statistics Office can issue certified copies of birth certificates for persons born in Florida from 1930 to the present.


Who Can Get a Birth Certificate?


  • Individuals 18 years of age and older may obtain their own certificate
  • Parents
  • Guardians who present court documents showing legal custody.
  • Attorneys representing children and adolescents under age 18 (documentation must be provided).


What Information is Needed for Birth Certificates?


  • A valid form of photo identification such as a driver's license, State ID issued by the Department of Motor Vehicles, passport or military ID. For mailed requests please provide enlarged, lightened, readable photocopies.
  • A completed Birth Certificate Request Form which is available in English and also in Spanish.
  • You will need the following information to complete the Birth Certificate Request Form:
    • Full name
    • Date of birth
    • County of birth
    • Mother's full name including maiden name
    • Father's full name (if applicable)

What is the cost of a Birth Certificate?


Each copy of a birth certificate is $15.00

Death Certificates

Who Can Obtain a Certified Copy of a Death Certificate?



Without the cause of death listed:

  • Anyone may request a copy of a Death Certificate without the cause of death showing on the certificate, it is considered a public record.

With the cause of death listed:

  • You must be a member of the immediate family. This includes a spouse or parent, child, grandchild, or sibling if of legal age
  • Beneficiary of a deceased person's life insurance policy that documents your beneficiary status
  • Estate administrator (must provide documentation)


What Information is Needed for Death Certificates?

  • A completed Death Certificate Request Form
  • You will need the following information to complete the death certificate form:
    • Full Name of the deceased
    • Date of death
    • County of death


What is the cost of a Death Certificate?


Each copy of a death certificate is $10.00

  • Your request will be processed immediately if you come to our office in person.
  • We accept cash (walk-in only) and money orders. Do NOT send cash by mail.
