Eligibility requirements at Manatee

Manatee County Health Department offers many
services that are available to anyone; other services require you
meet certain eligibility requirements prior to receiving the
service.

Services available to anyone (no eligibility
requirements, though some require financial screening and/or fees
for services):

Birth control (contraceptives)
-
Family Planning
-
Childhood Immunizations (vaccinations)
-
Immunization for foreign travel
-
Sexually transmitted disease (STD) testing,
treatment, and counseling.

Other services are available only to persons
determined to be eligible. To obtain a clinic card, you must
provide:

U.S. citizens: Government-issued photo
identification
-
Citizens of other countries in the U.S. on a
visa: Passport and I-94 form. Additional documentation may be
required.

Resident alients: Residency card (Green Card).
Additional documentation may be required.

Social Security cards for all in your household
-
Proof of current Manatee County residency (such
as a recent utility bill or rental agreement0
-
Proof of income for the last four weeks for
everyone in your household. (If you are self-employed, please
bring your most recent completed tax return).

We accept Medicaid or Medicare
-
Florida KidCare provides coverage for eligible
children under age 19.
