Death Certificates

The registrant's spouse
Parent
Child
Grandchild
Sibling, if of legal age...

Or to any person who provides a will that has been executed pursuant to s.732.502, F.S., insurance policy, or other document that demonstrates his or her interest in the estate of the registrant.

Please Note that:

Cause of Death cannot be included when requesting copies for probate issues.
Cause of Death is not needed for: real estate taxes, property claims, vehicles title transfer, closing out bank accounts.
Life insurance often times requires Cause of Death.

The fee is $6.00 for each certified copy (with or without the cause of death).

Applicant must submit a copy of his/her picture ID if requesting a certificate with cause of death and must indicate relationship to decedent. The identification must be in the form of a valid/unexpired photo ID in order to determine the applicant is the person they purport to be. The ID must be intact, in one piece, not torn or in more than one piece.

Birth Certificates

The Vital Statistics office issues certified computer generated birth certificates on any person born within the State of Florida from 1930 through the present. Birth certificates are confidential and can only be obtained by:

The registrant, if of legal age (Registrant - the child named on the record)
By the parents listed on the birth certificate
Or a legal guardian - must provide copy of guardianship papers

The fee is $15.00 for the first copy, $5.00 for each additional copy of the same record purchased at the same time.

Mail and Fax Orders

Print and fill out a birth application or death application and provide the following information:

A copy of a photo ID for a birth certificate
A copy of photo ID for a death certificate with the cause of death
When faxing requests, please be sure to enlarge and lighten the ID, if the ID is not legible the order will not be processed
A check, money order or Visa/MasterCard number and expiration date
Be sure to state your relationship to the registrant (person name on record)

Apostille or Exemplified Copies of Vital Records

Apostille or exemplified copies are usually needed for authenticating documents for use in foreign countries.

An apostille or exemplified copy of a vital record includes a statement signed by the Secretary of State attesting that the state registrar is the custodian of the vital records of this state as shown in the official records of the Department of State.
