Eligibility:

• Pregnant women or infants with a score of 4 or above on Healthy Start Screening Tool

• Pregnant women or infants determined at risk, with a score of less than 4 on the Healthy Start Screening Tool, that are referred by a health care provider

• * eligible for County Health Department Primary Care Services

Hours:

• Monday - Friday 8:00 a.m. to 5:00 p.m.
