Eligibility - WIC:

• 185 percent of federal poverty guidelines

• Pregnant, postpartum or breastfeeding women; infants up to 1 year of age or children up to 5 years of age with a medical or nutritional problem

• Referral from Health Department, private physician or social service agency

Eligibility - Nutrition:

• Medical nutrition therapy request must have written diet order from physician with diagnosis

• Referrals from Health Department or private physician

• Sliding scale fees

Hours:

• 7:15 a.m.-6 p.m. Monday-Thursday

• 7:15 a.m.-4:30 p.m. Friday
