Eligibility - WIC:

• At or below 185% of Federal Poverty Guidelines

• Pregnant, postpartum or breastfeeding women, infants up to 1 year of age or children up to 5 years of age with a medical or nutritional problem

• Referral from Health Department, Private physician or a social service agency

Eligibility - Nutrition:

• Medical nutrition therapy request must have written diet order from physician with diagnosis

• Referrals from Health Department or private physician

• None

• Sliding scale fees

Hours:

• Appt. hours - 7:15 a.m. to 6:00 p.m.  Monday - Thursday
                     7:15 a.m. to 4:30 p.m.  Fridays
