Community Medication
Assistance Program (CMAP)
Okaloosa County
The CMAP is provided through a partnership of the Okaloosa County
Health Department
and Sharing & Caring, Inc of Okaloosa County.
This program can help people navigate the complex process of
accessing medications through pharmaceutical companies'
compassionate medication programs. CMAP staff members help
facilitate completing and submitting the correct forms to request
medications to fill a valid prescription. Ultimately, CMAP
assists people (who do not have prescription drug coverage) get
prescription medicines at no or very low cost. |

• Anyone with a valid prescription written by
a medical provider
• Anyone who meets the income requirements of
the medication's marker (the pharmaceutical company) |

can help you find
out if you qualify for free or reduced cost
medicines from pharmaceutical companies. It
also helps you submit the correct forms to
pharmaceutical companies. |

WHAT YOU'LL NEED WHEN YOU COME TO THE OFFICE |
|
NOTE: In order to qualify, you
must have a primary care provider.
• A valid
prescription, written by a medical provider
• Proof of income (tax return, one month of pay stubs,
notarized letter of zero income, etc)
• Form of identification (driver's license, passport,
military ID, etc)
• Social security card |

About CMAP |

This program came to fruition
as a result of a significant community-wide health care need.
The CMAP partnership set sail to serve all Okaloosa residents as a
result of the
Homeless Community Deliberation Summit/Series, organized by the Greater
Fort Walton Beach Chamber of Commerce.
These meetings
include community members and representatives from local
businesses, and focus on ending homelessness in Okaloosa -- one life
at a time. |
