Vital Statistics Office

The vital statistics office provides Birth and Death Certificates.  Birth Certificates are currently available from this location for most individuals born in Florida.  Death certificates are available for Okeechobee County only.   For specific questions, please contact our Chief Deputy Registrar of Vital Statistics at: (863) 462-5804

We accept credit cards (Visa and Mastercard) for records purchased at the health department or by mail.  Please contact the vital statistics office for details.
