How to Request a Pasco County Death Certificate
WITHOUT CAUSE OF DEATH: Any person of legal age (18) may be issued a certified copy of a death record without the cause of death.
WITH THE CAUSE OF DEATH: Death records with the cause of death information may only be issued to the following individuals:
The Decedent's
1) spouse,
2) parent,
3) child, grandchild or sibling (if of legal age),
4)to any person who provides a will, insurance policy or other document that demonstrates his or her interest in the estate of the decedent, or
5)to any person who provides documentation that he or she is acting on behalf of any of the above named persons.
All requests for certification of a death certificate that includes the cause
of death information must include the signature of the applicant and his or her
qualifying eligibility. If after reading the above information you still are
uncertain regarding your eligibility for cause of death information, please call
our office, or our state office at (904) 359-6900, extension 9000 for
assistance.
All requests MUST include the following:
Decedent's:
Full name at death
Date of death
Place of death
Applicant's:
Full name, mailing address, daytime phone number.
Qualifying eligibility (relationship to decedent) spouse, parent, child, etc.
Copy of VALID PHOTO ID (Driver's license, State photo ID card, Passport, Military ID), when requesting Cause of Death information.
Applicant's SIGNATURE REQUIRED.
Orders will be returned unprocessed if the requests are incomplete.

Payment Methods:
The fee for each certified copy of a Pasco County death record is $10.00.
FOR WALK-IN REQUESTS, fees are payable by cash, money order or cashier's check.
FOR MAIL-IN REQUESTS, fees are payable by money order or cashier's check ONLY. (Please do not send cash by mail). Money orders/cashier's checks should be made payable to Pasco County Health Department.
Orders will be returned unprocessed if the requests are incomplete.
