How to Request a Florida Birth Certificate

 

Birth records can only be issued to:

1)             the registrant (the child named on the record) if of legal age (18),

2)             parent,

3)             guardian,

4)             a legal representative of one of these persons, or

5)             by court order.

 

All requests MUST include the following:

  

      Registrant's (person named on record):

Full name at birth, and any legal name changes (other than by marriage)

Date of birth

Place of birth (city or county)

Father's full name

Mother's full name including her maiden name

   

      Applicant's:

Full Name, Printed

 SIGNATURE REQUIRED

Qualifying eligibility (relationship to registrant)- Self, Mother, Father, etc.

 Mailing address

 Daytime phone number

Copy of VALID photo ID (Driver's license, State ID card, Passport, Military ID)

 

Orders will be returned unprocessed if the requests are incomplete

Payment Methods:

 

The fee for one certified copy of a Florida birth record is $12.00 and $8.00 for each addition copy of the same record when ordered at the same time.

FOR WALK-IN REQUESTS, fees are payable by cash, money order or cashier's check.

FOR MAIL-IN REQUESTS, fees are payable by money order or cashier's check ONLY. (Please do not send cash by mail).  Money orders/cashier's checks should be made payable to Pasco County Health Department.

 

Orders will be returned unprocessed if the requests are incomplete.
