Hours of Operation: Monday, Tuesday, Wednesday, Thursday from 7:30 am - 5:30 pm, Friday closed.
Appointments can be made directly with the receptionist at each facility or by phone.

Available Services:

 Exams, x-rays, cleanings, fillings, sealants, fluoride varnish and simple extractions (at doctor's discretion)

 Eligibility:

• Children ages 1 through 20 receiving Straight Medicaid, DentaQuest and DentaQuest Healthy Kids are eligible for our dental services.
• Healthcare Connection client with appropriate Dental Referral from Physician or Case Manager.

Medicaid and Healthy Kids will pay for dental services for your children.

A sealant is a material that is applied to the chewing surfaces of the back teeth. The sealant acts as a barrier, protecting enamel from plaque and acids.

 Fluoride varnish treatments (for children ages 1 to 20 years old) are provided twice a year to prevent cavities from forming on the smooth surfaces of the teeth.
