The Office of Vital Statistics is responsible for the recording and issuance of certified copies of birth and death certificates
To obtain and use a Florida Vital Record under false pretenses or for fraudulent purposes is a 3rd degree felony punishable by terms and conditions set forth in Florida Statute, S.382.026, F.S. The Putnam County Health Department is currently only able to provide Putnam County death records from 2009 to the present date. The Bureau of Vital Statistics in Jacksonville is diligently working on access to state death records for all county health departments in Florida. We will keep you updated on the release date.
How do I obtain a copy of a birth or death certificate from another state?
Other state's vital record information (birth, death, marriage or divorce) can be located at the following:
Birth Certificates
Florida birth certificates are available from 1916 to the present. On some occasions births are not able to be located in our system and will need to be referred to the Bureau of Vital Statistics in Jacksonville at (904) 359-6900.
Birth Certificates are confidential and can only be issued to:
- the registrant if 18 years or older
- parents listed on the birth record
- a legal guardian (must provide court documentation)
Forms available to print:
- Putnam County Birth Application
- Florida Birth Amendment Form (DH 429)
- Affidavit of Amendment of Certificate of Live Birth (DH430)
- Acknowledgment of Paternity (DH 432)
Photo Identification is required when requesting copies of birth certificates.
Acceptable forms of photo ID:
- State issued drivers license
- State issued photo ID card
- Military ID
- Passport
How to apply for a Florida birth certificate?
- In Person: Putnam County Health Department (Main Entrance) 2801 Kennedy St, Palatka, FL 32177. Bring photo ID and payment. Applications available at counter.
- By Mail: Putnam County Health Department, Attn: Vital Statistics, 2801 Kennedy St, Palatka, FL 32177. Send application, photo ID and a money order. Please note: for this request, there is a $10.00 mail fee. Can not deliver to P.O. Boxes, include the mailing address that FedEx can deliver to.
- Fax: (386) 326-3351 please fax application, photo ID, and return number to contact you for payment by phone. (Visa, MasterCard, and debit with Visa or MasterCard logo).
Fees:
- $12.00 per registrant
- $6.00 per additional copies of same registrant at same visit.
- $2.00 protective sleeve
- $10.00 rush fee
- $10.00 mail fee (fedex)

Death Certificates
Putnam County death certificates are available from 2009 to the present. If the death occurred in another county please contact that county health department or the Bureau of Vital Statistics in Jacksonville at (904) 359-6900.
Death certificates WITHOUT cause of death may be requested by any person.
Death certificates WITH cause of death are confidential and can only be issued to:
- the registrant's spouse
- parents listed on the birth record
- children of registrant if of legal age
- siblings
- grandchild
- any persons with legal documentation (will, insurance policy, etc)
Forms available to print:
Photo Identification is required when requesting copies of death certificates with cause of death.
Acceptable forms of photo ID:
- State issued drivers license
- State issued photo ID card
- Military ID
- Passport
