Immunizations

Immunizations are provided free for children ages 6 weeks - 18 years.

Adults (age 19 years and up) will be charged a fee.

Florida School Requirements:

The Immunization Guidelines (.pdf 745kb) for the State of Florida cover entry, attendance or transfer to preschools, public/non-public schools (K-12), licensed daycare facilities, and family day care homes. The school the child attends will have on file a Florida Certification of Immunization (also referred to as a "Form 680").
