Vital Statistics
Because of recent changes required to receive a Florida Drivers License, we are experiencing a higher volume of applications for Birth Certificates. Please allow a half day turn around when applying in person for your Birth Certificate. Our office hours are: 8:00am until 4:00 pm Monday through Friday.
Birth Certification Requirements
Years available: 1930 to the present (statewide)
Fees: $15.00 each
Issued only to the person named on the record, if 18 years of age or older, or the parent or legal guardian of the person named on the record (382.025 F.S.). Legal guardian must show guardianship documentation.
Applicants must have a picture ID
Payment in cash or check
Death Certification Requirements
Years available: 2007 to the present (for Suwannee County Deaths only)
Fees: $7.00 each
Applicants must have a picture ID
If cause of death is requested, must be an immediate family member of legal age or a legal representative representing immediate family member or estate of deceased (382.025 F.S.). Legal representative must provide legal documentation.
