Primary Care Services provided by
Taylor County Health Department
The Primary Care Program at Taylor County Health Department provides medical care and treatment for Taylor County residents. Treatment in Primary Care includes care of minor and acute conditions along with chronically ill clients who otherwise would not receive care because of lack of financial means and/or no insurance. Charges for services are calculated using a sliding fee scale taking into consideration the client's income. The Primary Care Program serves all ages, infant to aged adult who qualify financially for services. Open Access 
We now offer same day appointments through our Open Access Scheduling. Please call our office between the hours of 8:00AM to 9:00 AM to schedule your appointment. The number to call is (850)584-5087. We are located at 1215 North Peacock Avenue in Perry, Florida.
Medical Assistance Program
The Primary Care program also assists Taylor County residents with obtaining needed medications that they are unable to afford. Taylor CHD works with pharmaceutical companies to get medicines for patients, who qualify financially. We have a full time employee dedicated to work with patients from Taylor CHD and patients of other Taylor County doctors to help them secure the much needed medicines.
